MENTAL HEALTH Psychiatric care, case management, and counseling for individuals, families, and groups. Outpatient and residential options. ADDICTION & RECOVERY Evidence-based recovery management, counseling for substance use and gambling, medications for addiction treatment, and more. Available through outpatient care. PRIMARY CARE Preventive care, support for chronic conditions, and everyday health services. HOUSING Affordable housing, rent assistance, permanent supportive housing for people who need higher levels of care, and help finding emergency shelter. COMMUNITY WELLNESS Street and housing outreach, food boxes, hygiene supplies, movement, mindfulness, creative activities, and opportunities to connect with others. PEER-DELIVERED SERVICES Connection, support, and guidance from people with first-hand-knowledge of what it’s like to seek care and recovery. They can walk with you every step of the way.
