Green Country Health Innovations Inc The organization exists to provide palliative care for chronically sick individuals in the community it serves. The organization's palliative careactivities will be predominantly in-home or home setting support as defined by the Center to Advance Palliative Care and the National Hospiceand Palliative Care Organization. Palliative care will be 100 percent of the organization's activities, and these activities will be conducted by itsservice providers and/or volunteers. The organization's activities will be funded through donations from donors primarily focused on charitableactivities in healthcare. Palliative care furthers the organization's exempt purposes of providing relief for chronically sick individuals fromsymptoms and the stress of serious illness, improving the quality of life for these individuals, and facilitating potentially significant savings onhealthcare expenses in the community. As a nonprofit, nonhospital, the organization will be able to provide necessary treatment to
