IRONBOUND COMMUNITY HEALTH CENTER INC In 2024, we provided over 3025 clinic and behavioral health visits with onepractitioner and one mental health clinician. Over 80% of our patients thispast year were of Hispanic and Latino descent with almost 45% beinguninsured. Almost 50% live with chronic conditions such as diabetes andheart disease.HEC had been operating as a Department of Health and Human Services, HealthResources and Services Administration (HRSA) section 330 community healthcenter (CHC) having received $1 million dollars from HRSA for a ServiceArea Competition (SAC-AA) on 11/17/2020. As a new CHC from SAC-AA, theservice area was required to proceed through a SAC application process (FY2022 Service Area Announcement, HRSA-22-103) in the health center's firstyear of operation. HRSA's Objective Review Committee Final SummaryStatement scored our application with a raw score of 95, without thebenefit of additional priority points as a 1st year CHC.
