Healthy Community Forum for Sacramento Healthy Community Forum for the Greater Sacramento Region operates under the name Community HealthWorks (formally Sacramento Covered), a not-for-profit philanthropic organization, is an innovative leader in developing and implementing a peer-driven, one-on-one, data-informed approach to connecting families and individuals to the health, behavioral health, social services, and community resources they need to lead healthy, productive lives.
