Specifically, MCS offers the following services: ​ Home and Community-Based Services 245D Services Personal Care Assistance/CFSS Agency Model Homemaking Chore Services Waivered Transportation Non-Emergency Medical Transportation CFSS Consultation Services (start date October 1, 2024) MNsure Application Assistance Housing Stabilization Services ​ ​ For service referrals, complete this form and return to us via email at info@mcsmn.com or fax at 952-777-2263. ​ Further, MCS is a certified Essential Community Provider through the Minnesota Department of Health. As an Essential Community Provider, MCS provides reduced or no-fee services, based on its sliding fee scale.
